Eliot Community Human Services Case Manager (Community Support Program) in Lynn, Massachusetts
Eliot is searching for Case Managers to work with clinicians to develop and implement a goal-oriented service plan. They would participate in all service planning and interagency Planning Meetings chaired by the IntensiveClinical Management Clinician. The ideal candidate will be able to establish effective working relationships with clients and utilizes solution based interventions which best address specific problem area as identified in the client’s treatment plan.
Our multi-disciplinary team of psychiatrists, psychologists, social workers, mental health counselors and other professionals administer a full range of clinical and support services, resulting in a comprehensive collaborative approach to treatment.
Contact newly referred consumers within 24 hours of referral by the Intensive Clinical Management Clinician of the contracted payer to complete biopsychosocial assessment and initiates service planning.
Participate in all service planning and interagency Planning Meetings chaired by the Intensive Clinical Management Clinician of the contracted payer.
Develop and implement a goal-oriented service plan in accordance with Quality Improvement Standards, sound practice, and in collaboration with consumer and treatment team.
Facilitate consumer treatment compliance and community stability through use of aggressive outreach, education, support, hands-on assistance and teaching consistent with identified service provision goals.
Reassess consumer needs on an ongoing basis and conduct concurrent reviews with insurers as needed to obtain authorization for services.
Document all clinical and collateral contacts as well as case management activities in the medical record on the day of service provision.
Provide liaison between the program and other agencies to insure the integration and coordination of services for the consumer. Advocate for consumers with agencies such as Visiting Nurse Association, Home Care agencies, mental health and substance abuse providers, hospitals, and other natural support systems and generic services through visits, letters, and phone calls to insure availability and accessibility of services.
Understand utilization expectations; and submit all billing in a timely manner.

- Bachelors or better in Psychology
Licenses & Certifications
- MA DRIVER'S LICENSE
- 1 year: Working with Adults with persistent and severe mental illness.
- Understanding of public and private health insurance plans.
