Eliot Community Human Services Homeless Housing Coord Supervisor in Lynn, Massachusetts
Description
Eliot is in search of a bachelor's level person with at least a year of supervisory responsibilities in the human services realm. Eliot’s Permanent Supportive Housing Teams provide multidisciplinary trauma informed/strengths based services to effectively address barriers to obtaining and maintaining housing (eg untreated PTSD/Mood Disorder(s)/Psychosis/Substance Use Disorder(s)/medical complexity)
Responsibilities:
Ensure that consumers are treated with dignity and respect in accordance with ECHS’ Human Rights Policy.
Conduct client interviews and perform assessments of clients experiencing homelessness, make collateral contacts, and consult with other providers (i.e., mental health, primary care, including services to address substance use disorder including recovery coaching and/or medication assisted treatment)
Assess client needs and seek consultation as well as determine the resources required to ensure comprehensive service delivery.
Perform housing search in the Boston (and Greater Boston areas) for clients who have a voucher, and work to secure client placement in housing
Maintain frequent contact with property managers and landlords (including Boston Housing Authority) to determine appropriate services for residents of program, and to ensure a positive relationship with housing providers
Evaluate clients’ capacity to make independent living decisions, including risk assessments for clients around safety and tenancy preservation
Develop and modify individual client service plans to assess ongoing needs for clinical, supportive services and housing needs
Schedule: Monday - Friday 9am-5pm
