Position: - Residential Services Direct Care

Reports to: Program Manager

Classification: Non-Exempt

Location: Lawrence Street

Hours: Full Time 38 hrs. Tues 3pm-11pm, Wed 4pm-9pm, Th 3pm-11pm, Fri 4pm-11pm and Sat 11am-9pm.

Position Overview:

It's an exciting time to join our Residential Services division. If you love helping people and have worked/trained in disability services before, this may be the role for you! This role will work to support individuals in one of our 9 residential homes.

Primary Responsibilities:

  • Provide case management for assigned individuals. Provide direct care coverage and support for all individuals served, responding to individual issues during assigned shifts.

  • Develops plan, in conjunction with Dr., occupational therapist, speech therapist, etc.

  • Work with individuals to promote growth in each of QUEST's quality of life areas (i.e., Individual Control, Respect and Dignity, Relationships, Community Membership, Personal Growth & Accomplishments, Personal well-being).

  • Assist individuals in working on areas identified in their Individual Support Plans (in conjunction with the efforts of other team members).

  • Support individuals as needed or directed, in the areas of problem solving, personal care, medical care, socialization and leisure activities, interpersonal skills, household, money management, activities of daily living, asserting their human and civil rights.

  • Know and adhere to all safety policies and emergency procedures. Know basic emergency facts regarding all individual living at the residence.

  • Dispense medications as prescribed. Report any medication occurrences to Program Manager, complete necessary forms and follow applicable procedures.

  • Complete any necessary documentation, including (but not limited to) Communication Log, Case Log, ROADs Charts, Daily Progress Notes, ISP Assessments, Support Strategies, Incident Reports, Medication and Treatment Charts, Medication Occurrence Reports.

  • Plan/Transport/Accompany individuals to appointments and other community-based activities.

  • Attend and participate in supervision and staff meetings as scheduled by the Program Manager.

  • Assist in the development and implementation of behavior support plans.

  • Provide instruction and guidance to individuals' families regarding individual program plans and procedures.

  • Maintain regular contact with the DDS Service Coordinator and families of assigned individuals.

  • Maintain regular contact with day programs, therapist and others involved in supporting assigned individuals to ensure continuity of supports.

  • Reports, Medication and Treatment Charts, Medication Occurrence Reports, Communication Logs between day/residential supports, Money Counts, etc.


  • Bachelor’s Degree in related field preferred.

  • One to two years’ related experience and/or training; or equivalent combination of education and experience.

  • CPR/First Aid Certificate

  • Valid MA license, availability of vehicle

Tuesday: 3pm-11pm

Wednesday: 4pm-9pm

Thursday: 3pm-11pm

Friday: 4pm-11pm

Saturday: 11am-9pm
