Advocates Jail Diversion Program (JDP) Community Resource Navigator in Massachusetts

Overview

$21.00-$23.00 /hr

TheJDP Community Resource Navigator will provide case management, and substance recovery support to individuals and families involved with JDP. Navigators will work with program participants to identify needs and strengths, and develop/implement person-centered treatment plans that will support achievement of their goals, reduce risk and barriers, and support engagement in appropriate levels of care.

Responsibilities

  1. Provide program participants with supportive outreach, psychoeducation, and other needed services.

  2. Use evidence-based methods to complete required assessments, offer counseling and support services, and provide supported referrals to community resources as identified.

  3. Work collaboratively with Program Coordinator or other designated clinical staff to interpret assessment findings, develop an individualized Service plan (ISP), and identify community based supports that coincide with participant goals.

  4. Actualize and implement treatment plans for assigned program participants. Meet with assigned participants on weekly basis or a designated time frame within each ISP to review and document progress in accordance with program requirements.

  5. Coordinate community-based services as required.

  6. Accurately record and document all meetings and interactions with service recipients in their file to reflect individual’s progress towards obtaining goals and desired outcomes in accordance with program requirements.

  7. Maintain required data, notes, and other necessary records and documentation in accordance with program. Review efforts on a weekly basis with Program Coordinator or other designated clinical staff.

  8. Complete all documentation, file notes, updates and assessments within the timeframe established by the program and/or Advocates.

  9. Support the strength-based, client-driven model by empowering participants in conflict resolution and decision-making.

  10. Coordinate communication with multiple agencies and staff who are project partners, including all key clinical and other support providers, probation officers, and other partners as they are identified.

  11. Responsible for conducting meetings with participants to conduct initial intake and begin preparing resources based on the participants’ reported and observed needs.

  12. Provide transportation to participants, as appropriate and necessary, within caseload for the purpose of eliminating barriers, introducing participants to other clinical or support providers, or fulfilling other needs as deemed appropriate by Program Coordinator.

  13. Attend and actively participates in supervision, staff meetings, multidisciplinary team meetings or meetings related to participant specific needs or services.

  14. Attend all trainings as assigned and maintain current all required certifications.

  15. Make timely connections to emergency services or crisis intervention as needed.

  16. Assist with facilitating the most appropriate referral to the appropriate level of care and/or agency for individuals in collaboration with the team.

  17. Develop and maintain a thorough knowledge of managed care and health insurance systems; provide other staff and clinical or support providers with managed care information and resources pertinent to area of need.

  18. Assist in communicating with Co-Response and Police regarding cases, as appropriate.

  19. Remain alert at all times throughout the scheduled shift.

  20. Adhere to all principles related to the Advocates Way.

  21. Ensure that all participants are treated with dignity and respect in accordance with Advocates’ Human Rights policy.

  22. Perform all duties in accordance with the agency’s policies and procedures.

  23. Strictly follow all agency Performance Standards.

Qualifications

  1. Bachelor’s Degree in social work or in human services or health care related field with at least one to two year’s minimum experience working with individuals in a crisis or community-based support setting.

  2. Ability to develop and maintain satisfactory working relationships with participants and community providers.

  3. High energy level, superior interpersonal skills and ability to function in a team atmosphere.

  4. Must adhere to all professional ethics.

  5. Ability to communicate effectively verbally and in writing.

  6. Must have basic computer skills.

  7. Ability to read and speak English.

  8. Commitment to Advocates values and mission.

  9. Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.

  10. Must hold valid driver’s license and access to an operational and insured vehicle.

  11. Must be able to perform each essential duty satisfactorily.
