Clinician Consultant - Relief

We are seeking a relief Clinician Consultant for our Adult Community Clinical Services (ACCS) program.

ACCS provides wraparound support through an integrated team of both clinical and peer professionals. The goals of the service include skill and resource development, connection with community and natural supports for successful tenure in the community of the individual’s choice. The Clinical Consultant is a per diem position, providing care coordination through development of a collaborative relationship with the person receiving services, and connections with family, other providers and community supports that are important to the person.

The Relief Clinician Consultant will also:

  • Provide evidence –based interventions to provide person-centered assessments and service planning, and advocacy, coordination of care, and linkage with other services and the community.

  • Emphasize skill building that focuses on sustained engagement, independent housing, community tenure that is uninterrupted by hospitalizations, symptoms stabilization, self-management.

  • Serve as the clinical point of contact for all supplementary supports, including Care Coordination entities.

    Rate: $40.00/hour

    Schedule: Relief hours available

Required Skills

  • Must have excellent written and verbal communication skills (in the English language) and the ability to make effective public presentations.

  • Must have valid driver’s license and available, dependable transportation and be willing to provide transportation to person's served in personal vehicle, as needed.

Required Experience

  • Master’s degree required

  • Licensure required within 2 years of hire: LICSW or LMHC

  • Must have previous experience working in ACCS
