High Risk Patient Program Care Manager for Milford Regional Physician Group ( Statement of Purpose: The High Risk Patient Program Care Manager is responsible for establishing, implementing, monitoring and evaluating high cost effective care plans for a designated group of patients in the ambulatory setting. Utilizing a patient centered approach, the care manager directly interfaces with physicians, health care teams, patients and their care givers in managing the patient's care. Qualifications:

Current RN license with a minimum of 2 years of clinical experience required. 1 year of previous experience as a care manager required. Excellent computer and technology skills are also required. Previous experience with MS applications, virtual platforms, and EMR systems strongly preferred. Schedule:

Monday-Friday, 8:00am - 4:30pm or 8:30am-5:00pm
