Intensive Care Coordinator - Bachelor's Level
Exciting opportunity to join our innovative and growing children's mental health program as a Bachelor Level Intensive Care Coordinator!
We are seeking a compassionate and culturally sensitive professional who is dedicated to making a positive impact on the lives of others and thrives in a diverse and collaborative environment!

Responsibilities:
Provide comprehensive care coordination to children and families.
Perform initial clinical assessments to identify family and youth's need and goals.
Develop, implement and monitor individualized care plans based on family needs and goals.
Facilitate care plan team meetings.
Act as a liaison with collateral agencies and programs
Advocate for families and educate them on self-advocacy
Identify and assist youth and families in accessing necessary services
Ensure proper documentation, and safety planning
Participate in staff meetings, supervision, and training
We offer an amazing orientation and well-structured training in high fidelity wraparound!
Rate: $22.28/hourly
Schedule: Full Time, 40 hours

Required Skills
Excellent communication and organizational skills
Basic computer fluency, including Microsoft Office
Valid driver’s license and reliable vehicle
Bilingual in Spanish or Portuguese highly preferred
Required Experience
Bachelor's degree in a mental health-related or human services field (such as social work, counseling, child development, psychology, human services) from an accredited college or university
Minimum of one-year experience working with children with serious emotional disturbance
Community-based experience strongly preferred
