Job Summary:As a member of the Interdisciplinary team coordinates patient care through the continuum thereby facilitating the achievement of optimal quality outcomes in relation to clinical care and cost-effectiveness. Provides resource counseling and discharge planning services to patients/families in inpatient and outpatient settings Identifies high-risk psychosocial factors of patients/families that impact discharge planning/health status including changing issues, needs, and priorities along courses of illness and end of life care. Helps hospital staff understand the influence of those factors upon the course of medical care. Assumes care coordination responsibilities for patients with complex psychosocial risk (domestic violence, protective services, frail elderly, disabled, substance abuse, etc.). Assesses adaptation of families to patient's illnesses, provides crisis intervention as needed, and Serves as an expert about and as a liaison to community resource programs. The role is based on professional standards of practice of the National Association of Social Work Code of Ethics.

Essential Responsibilities:

  • Identifies patient's/family's psychosocial, financial or legal concerns that affect patient care management and interacts with patient/family/interdisciplinary team to facilitate the patient care process

  • Provides resource counseling and discharge planning to patients/families during admission and in outpatient settings

  • Identifies aftercare options for individual patients in conjunction with utilization review

  • Identify and address barriers to discharge; locate resources; identify options and available supports; facilitate referrals and applications to government/community agencies; advocate for access to resources; coordinate referrals and/or placement plans; assist patient and family to emotionally prepare for transitions; prevent readmissions for non-medical reasons

  • Ability to case manage complex situations so that continuity of care is maintained

  • Ability to assess and reassess impact of interventions with clients, families and interdisciplinary team members to modify and develop care plans accordingly

  • Ability to identify changing issues, needs, and priorities along courses of illness up to an including discussions about end of life care

  • Ensures coordination of the communication process with patient/family concerning the discharge plan including coordination of family meetings

  • Participates in daily rounds with interdisciplinary team on nursing units

  • Maintains communication with physicians, nursing personnel, ancillary services to help facilitate optimal ongoing patient care

Required Qualifications:

  • Master's degree in Social Work required.

  • License Lic Indep Clinical Social Work required., or Registration Licensed Social Worker required.

  • 1-3 years related work experience required in an acute care hospital setting, working within an interdisciplinary team..

  • Experience with computer systems required, including web based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access.

  • Independence of Action:Ability to follow precedents and procedures. May set priorities and organize work within general guidelines. Seeks assistance when confronted with difficult and/or unpredictable situations. Work progress is monitored by supervisor/manager.

  • Written Communications:Ability to communicate clearly and effectively in written English with internal and external customers.

  • Oral Communications:Ability to comprehend and converse in English to communicate effectively with medical center staff, patients, families and external customers.

  • Knowledge:Ability to demonstrate full working knowledge of standard concepts, practices, procedures and policies with the ability to use them in varied situations.

  • Team Work:Ability to work collaboratively in small teams to improve the operations of immediate work group by offering ideas, identifying issues, and respecting team members.

  • Customer Service:Ability to provide a high level of customer service to patients, visitors, staff and external customers in a professional, service-oriented, respectful manner using skills in active listening and problem solving. Ability to remain calm in stressful situations.

Age based Competencies:

Employees in this job must be competent to provide patient care to the following age groups: Neonatal:Birth to 6 months, Youth: 6 months to 16 years, Young adult: 16-30 years, Middle Age: 30 - 60 years, Elderly: 60 -.

Physical Nature of the Job:

Light work: Exerting up to 20 pounds of force frequently to move objects. Some elements of the job are sedentary, but the employee will be required to stand for periods of time or move through out the hospital campus
