POSITION RESPONSIBILITIES: Reviews the Screening, Assessment, Crisis Plan, Critical Needs Plan and IAP upon completion or update completed by the LPHA. In conjunction with the Integrated Team/GLE/IGLE staff, assists persons with identifying and implementing meaningful goals for their Individual Action Plan focusing on the following areas: mental health recovery education, daily living skills, substance use, relationships, independent transportation, self-help groups, exercise/wellness groups and the Recovery Learning Center (RLC), vocational & leisure. Demonstrates knowledge of prescribed medications, ability to monitor medications, and follows FHR state specific medication guidelines. Organizes, plans, and facilitates a range of valued community activities that offer wellness/fitness, social, and cultural opportunities outside of the persons\' home. Offers or assists with transportation needs, supporting engagement/participation in recovery focused community resources. Maintains a positive attitude and promotes an environment that is recovery based and supports the individual rehabilitation goals and interventions of consumers that support hope, healing and empowerment. Works with person to identify and utilize preferred recovery tools and self care techniques (i.e. WRAP, peer groups, etc.). Demonstrates appropriate therapeutic boundaries. Provide face to face crisis intervention to aid the person in managing a crisis, providing short-term intervention to decrease risk, manage symptoms, and maintain safety. Carries out routine daily tasks including but not limited to scheduling/ rescheduling or attending doctor\'s appointment, shopping, entitlement/housing support, court, etc. Shares and encourages persons to participate in the completion of domestic tasks, assisting in the overall maintenance/upkeep of the program/supported living offering a safe clean and comfortable and safe environment for treatment to take place. Shows consideration and flexibility to support scheduled and emergent needs that occur within the ACCS system. QUALIFICATIONS, EDUCATION, AND EXPERIENCE: High School Diploma or GED One year full-time experience in providing direct care service to mentally ill consumers preferred. Preference will be given to multi?lingual candidates who meet the cultural and linguistic needs of the population served.
