Eliot's Homeless Division is seeking a dynamic, energetic Clinician to fill their Team Lead Position. The Homeless Division Outreach begins with the Federal and State funded Project for Assistance in Transition from Homelessness (PATH), which provides person-centered and trauma-informed support services to individuals with serious mental health illnesses, as well as those with co-occurring substance use disorders, who are homeless or at risk of becoming homeless. Outreach Clinicians and Engagement Specialists engage, assess, and collaboratively develop a recovery-oriented treatment plan that incorporates identified needs and preferences of the person served. Service referrals may include but are not limited to:
Primary Care/Accountable Care Organizations (ACO);
Access to federal and/or state benefits (social security, legal services, health insurance, transportation, food);
Behavioral Health (Psychiatry/Counseling);
Specialized Therapy to address PTSD;
Substance Use Disorder Supports: Recovery Coaching, Medication Assisted Treatment, Harm/Risk Reduction, Rehabilitation, Sober Living, Detoxification;
Department of Mental Health Services; and Intensive Case Management.
Responsibilities:
Promote hope, empowerment and recovery in all aspects of care. Model professional behavior for staff and promote use of recovery based language.
Provide overall clinical supervision and oversight of service delivery of the team.
Provide clinical guidance and support in developing interventions and supervision of team members.\
Provide direct supervision to all staff both in the office and in the field. Teach and model clinical interventions, strength based strategies and ensure services are culturally competent.
Participate in the hiring of staff for all programs and services in assigned area.
Conduct annual performance evaluations of assigned staff.
Review eligibility and assessment documentation and provide consultation, ensuring compliance with SAMSHA and state requirements as well as policies and procedures.
Schedule: Monday- Friday, full time 40 hours per week
