Riverside Community Care Case Manager - Jail Diversion in Norwood, Massachusetts

We are seeking a BH-JI Navigator/Recovery Coach - Jail Diversion for our Connect2Recovery program!

Navigators provide case management and system navigation supports to Mass Health eligible justice involved individuals struggling with a diagnosis of substance use disorder and/or co-occurring mental health disorders. The purpose of BH-JI services is to engage individuals while in correctional institutions and assist them in successfully transitioning into the community upon release, including supporting them in accessing treatment services, community resources, and recovery-supported peer/social supports. In addition, the Navigator will assist the person in identifying and working through system barriers to recovery and healthy community living.

Essential duties and responsibilities include but are not limited to:

  • Provides information about, and facilitate access to, community and recovery supports, including supports for families

  • Develops a set of goals and objectives in conjunction with the person, based on needs identified by them and/or other providers.

  • Supports the person served to achieve their own unique pathway of recovery, which may include harm reduction approaches, Medication Assisted Treatment, 12-step, and other methods of recovery.

  • Completes all clinical and administrative documentation, including progress notes, incident reports, in a complete, accurate and timely manner and in compliance with agency and relevant state protocols.

Required Skills

  • Bachelor’s Degree in mental health related discipline strongly preferred; Associate’s Degree or High School Diploma/GED considered with at least 2 years of related professional experience and/or personal experience

  • Certification as a Recovery Coach preferred

  • Must be vetted to be able to enter and provide services in correctional institutions.

  • Valid driver’s license, safe driving record and regular access to a reliable vehicle required.

Required Experience

  • Excellent communication, interpersonal, and communication skills

  • Basic computer/technology fluency (including Microsoft Office; comfortable navigating an electronic healthcare system – EHR.) required

  • Navigators must possess sufficient knowledge and understanding about treatment and recovery from substance use disorders, mental health disorders and/or criminogenic issues. This includes but is not limited to: an understanding of addiction, mental health and criminogenic services available for Mass Health Members and resources available in the geographic area where they will work.
