MOLARI Employment & Healthcare Services Assessment Stabilization Representative in Pittsfield, Massachusetts

We are seeking an Assessment Stabilization Representative for a company in Pittsfield. This position will be responsible for the implementation of initial, and when appropriate, in-depth screening and assessments of all referred customer households. Job Classification: Temp-to-Hire, Full Time Work Schedule: M-F, 8am-4:30pm Salary: $37,000 per year Responsibilities: Assessment/Triage Activities: Conduct a preliminary and if necessary, in-depth Housing Needs Based Assessment to best determine eligibility for financial assistance when appropriate and available. Identify resources and services as requested and/or needed to address barriers to housing and assistance with financial applications Develop partnerships and collaborations with internal and external service providers to ensure customer receives comprehensive services Work collaboratively with other internal staff to assist customer with move-in transition supports including but not limited to; furniture and furnishings assistance, budget support, and other resources and services as needed Intake Activities: Conducts initial customer intakes, inform customers about available resources and related program protocol and requirements as well as other internal programs, obtains necessary background information from customers and track customer cases from opening to closing. Maintains electronic records, in accordance with agency and agency's funders' standards and requirements and works to assure confidentiality of said records. Develops, maintains, and uses an accurate, up-to-date resource file that contains information on available community resources including data on similar and/or related services that may address the needs of persons seeking assistance with housing, homelessness, and related issues. Assists with outreach activities including but not limited to contacting other social service agencies and clients regarding services and programming available. Housing Stabilization: Track all families receiving HomeBASE or other financial assistance on a monthly basis to ensure stabilization assessments are completed and that family remains successful with their Housing Plan. Ensure that subsidized households receiving HomeBASE assistance receive monthly check-ins and lease compliance checks; and non-subsidized households receive monthly checks-ins and ensure that progress reports are completed on indicators of moving towards self-sufficiency. Ensure that all data requirements for each individual financial program are fulfilled in a timely manner and all reports are submitted within indicated time frames. Keep individual, detailed and confidential case files on each household receiving stabilization services Requirements: High school diploma; Associates Degree preferred in Social Science, business administration or a related field Experience with administrative work and/or training which provides the required knowledge, skills & abilities to perform the duties of the job Valid driver's license/Reliable transportation Microsoft Word, Excel, Outlook Basic internet understanding Excellent oral and written communication skills Qualifications: We will train candidate in MA state service systems including but not limited to, Department of Transitional Assistance, Department of Housing & Community Development, The Department of Children & Families and the Department of Mental Health. Knowledge and experience with service agencies and resources for homeless individuals and families Experience with direct service delivery; Willingness to work as part of a team to promote the goals of the program and agency Sensitive to the needs of the homeless, low income and diverse populations Ability to work in a busy, diverse team setting Excellent time management, organizational and communication skills Excellent oral & written communication Bi-lingual preferred
