Center for Human Development Enrollment Specialist in Pittsfield, Massachusetts

Receives enrollee referrals, electronically and telephonically from MassHealth, ACOs, MCOs, and community providers - Reviews EHR portal throughout the day and develops a revolving call schedule to ensure that enrollees are assigned a Care Coordinator within five business days - Verifies eligibility for CP services with MassHealth or the applicable ACA - Applies decision tree to assign enrollees to appropriate CP treatment teams - Initiates recruitment contact to enrollees, either by phone calls or conducts outreach visits to enrollees? homes or community settings - Utilizes multiple vehicles in order to obtain updated contact information for enrollee - Introduces enrollee to the ICP program - Assigns enrollees using established protocols and criteria - Responds to enrollee questions and concerns in a timely manner, provides enrollee handbook and has enrollee review, understand and sign participation form and any other Community Partner program information - Verifies demographic and insurance information of enrollees - Establishes electronic health record for enrollees - Schedules initial visit for Care Coordinator - Facilitates communication between enrollee and care team members Expectations: Outreaching to various communities - Connecting with different agencies to assist with enrollment - Signing up enrollee?s on our assignment list - Knowledge of ICP and being able to talk about it. - Ability to work with people from different backgrounds.
