Jordan Hospital Resource Specialist I - Community Case Management in Plymouth, Massachusetts

Duties/Responsibilities:

  • Works closely and collaboratively with the Program Manager and Resource Specialist 2

  • Responsible for daily support of the patient identification, screening and assignment process

  • Utilizes multiple software and EMR systems to identify patients as being enrolled/dis-enrolled from the program

  • Assists with the tracking of program metrics

  • Performs quality control review for follow-up and tracking of program patients

  • Assists with maintaining and updating the complex case management database, electronically and/or by spreadsheet

  • Provides concise documentation, in the case management software database, for all tasks completed

  • Assists with maintaining the Resource Manual

  • Provides some patient follow-up as indicated/directed

  • Assists Resource Specialist 2 with identifying patient resources, as needed

  • Functions as an integral member of the Community Case Management team

  • Communicates, ongoing, with the CCM team

  • Function as an active member at team and other program meetings

  • Identifies, analyzes, and provides resolution of issues to support the team workflow

  • Participates in interdisciplinary continuous quality improvement activities on an ongoing basis

  • Demonstrates a commitment to the program goal of improving the quality of care provided to patients

  • Work requires reading, writing and checking documents for accuracy, opening attachments and inputting/editing words or data into or from a computer system (Allscripts, Meditech, Athena, ECW, eCentricity, Microsoft Office, Word, Excel)

    Education/Experience Required:

  • Bachelor’s degree in a health care related field strongly preferred

  • Experience in health care field required

  • Excellent interpersonal and communication skills, including phone etiquette and written communication skills

  • Strong understanding of medical terminology

  • Must be able to work independently and as part of a comprehensive team

  • Excellent computer skills; Microsoft Office Suite

  • Strong organizational skills; Detail oriented
