State of Massachusetts Clinical Social Worker (A/B) in Pocasset, Massachusetts
Work Schedule: (40 hours) Monday through Friday 8:00AM - 4:30PM
Days Off: Sunday and Saturday
The Department of Mental Health's Pocasset Community Mental Health Center is seeking a dynamic individual to fill a Clinical Social Worker position. The Clinical Social Worker, provides therapy to individuals and families; carries out specifically assigned tasks related to treatment plans for patients; reviews patients progress; recommends the referral and arranges for the transfer of patients to other programs or facilities; provides social work services to adults under the direct supervision of a Clinical Social Worker, and provide therapeutic groups on In-Patient Unit.
DETAILED STATEMENT OF DUTIES AND RESPONSIBILITIES:
Obtains information through interviews with patients, relatives and community-based mental health professionals to provide psychosocial assessment for treatment planning.
Actively participates in interdisciplinary treatment conferences in order to present psychosocial data for the purpose of developing and reviewing treatment plans.
Provide therapeutic groups to clients on 16 bed psychiatric inpatient unit and documents in accordance with policies/procedures.
Provide therapy and service information to patients and their families for the purpose of increasing patients level of functioning to educate families regarding the illness of their family members.
Coordinates discharge planning for patients by means of treatment planning conferences involving patients, relatives, community caregivers and team members in order to insure the most appropriate living and aftercare arrangements.
Participates in supervision on a regular basis and through case review and discussion is encouraged and supported in further improving treatment skills especially as related to adults.
Service as a social work resource to assist in obtaining patients medical benefits.
Continues to improve skills through the completion of all mandatory staff development classes and participation in social work related continuing education programs.
Prepares and maintains case records and narrative, statistical and team meeting reports and other required documentation in both automated and written form.
Completes psychosocial assess and other related assessments within established time frames.
Demonstrates the knowledge and skills necessary to provide appropriate care to the age related, cultural and spiritual needs of the patients served.
Other related duties as assigned.
Given the population served, bilingual or multilingual fluency.

MINIMUM ENTRANCE REQUIREMENTS:
REQUIRED WORK EXPERIENCE: None. (See Required Education and Licenses.)
REQUIRED EDUCATION: A Master's or higher degree in social work.
LICENSES: Based on assignment, travel may be required. Those employees who elect to use a motor vehicle for travel must have a current and valid Massachusetts Motor Vehicle Operator's license or the equivalent from another state.
- Current and valid licensure as a Licensed Certified Social Worker or Licensed Independent Clinical Social Worker by the Massachusetts Board of Registration in Social Work.
