State of Massachusetts Case Manager - Child & Youth Division in Quincy, Massachusetts

The Child Youth and Family Services operated by the Department of Mental Health (DMH) at the Quincy Site Office is seeking a dynamic candidate who will provide Case Management services for eligible youth under age 22, with majority of caseload below age 18. The successful candidate will have knowledge and desire to work with youth with mental health need and their families to promote recovery and connection to community-based services. The Quincy Site Case Manager additionally is instrumental in interfacing with schools and other child-serving agencies to provide collaboration and education on youth mental health and available services. _Please Note:_The work schedule is* Monday through Friday 8:30 a.m. to 5:00 p.m. (40 hours/week) with Sunday and Saturday off. *_Duties and Responsibilities: (these duties and responsibilities are not all inclusive):_ * Provide case managementservices for eligible youth under age 22, with majority of caseload below age 18. * Position will be required to conduct comprehensive child centered family focused assessments, participate in service and treatment planning and * Develop disposition plans for youth that focuses on community-based services. * Work with a team of internal and external providers and family to ensure integrated planning and service provision. * Ensure all documentation in clinical records and billing documents are completed. * Access community resources and monitor service provision with contracted providers. Youth will be involved with multiple systems such as school, outpatient treatment, DCF, DYS and MBHP services. * Monitors the implementation of Individual Service Plans (ISP) * Consults with service providers and community agencies on Child, Youth and Family Case Management issues; providing and/or arranging transportation of clients; and attends and participates in client service planning and discharge planning. * Plan, convene and chair meetings of client’s care planning team to discuss ISP effectiveness and modifications, coordinate service delivery and other assistance. _Required Qualifications:_ * Knowledge of psychosocial problems, types and symptoms of those with mental and emotional disorders. * Knowledge of the principles, practices and techniques of counseling, interviewing, crisis intervention, motivation and reinforcement. * Knowledge of the types of programs and services available to the mentally ill population. * Ability to understand, explain and apply the laws, rules, regulations, policies, procedures, specifications, standards and guidelines governing agency and Site activities. * Ability to make case assessments by examining records and documents and through questioning and observing clients. * Ability to communicate effectively orally and in writing, giving clear accurate instructions. * Ability to make decisions, act quickly and maintain a calm manner in stressful or emergency situations. _ Special Requirements:_ Travel is required - This position directly manages cases in the DMH Quincy Site catchment areas and services in which will be primary travel locations; however, frequent travel is required throughout the DMH Southeast Area and occasional required statewide travel to various acute inpatient hospitals or other treatment settings. Current and valid Massachusetts Class D (or equivalent from another state) Motor Vehicle Operators license required. _ Preferred Qualifications:_ * 3 years' experience in child/adolescent services. * Multilingual fluency. _ Mission Statement:_ Department of Mental Health provides Child, Youth, and Family Services to eligible individuals who meet the clinical criteria for service authorization.
