ESCCI Core Values Advocacy Collaboration Compassion Diversity Innovation Position Overview: The Geriatric Support Services Coordinator is responsible for coordinating support services as part of the Senior Care Options Primary Care Team. The Geriatric Support Services Coordinator?s basic function is to assist eligible SCO & One Care members to obtain services which will enhance their quality of life. Duties & Responsibilities: Perform initial & ongoing assessments of the health & functional status of the member. Assist in the development of the member?s community-based care plan & related service package. Track member transfers from one setting to another, work to adjust the Individual Care Plan as appropriate. Coordinate direct services to members. Assist members with a Loss of Mass Health. Meets monthly with SCO/One Care team members to review cases. Maintains case records as required by the Senior Care Organization, One Care Organization, Agency, & the Executive office of Elder Affairs. Participates in in-service trainings, courses, & workshops. Participates in special initiatives & projects within the agency. Other duties as assigned. Skills & Abilities: Ability to collaborate, take initiative, provide/ receive feedback. Excellent computer skills required. Ability to work independently & professionally. Ability to communicate & work with the elderly & community resources serving the elderly population. Valid MA driver\'s license & car required Education, Licensure & Experience: A Bachelor?s degree in social work or related human services field from an accredited college or university. Physical Requirements: Prolonged periods sitting at a desk & working on a computer. Must be able to lift up to 15 pounds. Travel required throughout the service area.
