Home Care Director The Loomis Communities South Hadley, MA Position Overview: Loomis At Your Door is seeking an experienced and visionary Home and Community-Based Services Director to lead and grow our Home Care Program. This role involves ensuring residents receive the most appropriate services aligned with their wishes and needs, while maintaining the highest standard of care. The ideal candidate will demonstrate strong leadership skills, manage a competent and compassionate team, and develop a strategic vision for maximizing program growth and revenue. Key Responsibilities: Leadership & Vision: Develop and implement a strategic plan for Loomis Home Care that aligns with organizational goals and supports program growth and financial sustainability. Team Management: Oversee the hiring, orientation, supervision, and evaluation of the Home Care team. Provide ongoing training, education, and performance management to ensure adherence to Loomis? Guiding Principles. Quality Assurance: Initiate quality improvement activities based on Key Performance Indicators to maintain and enhance service excellence. Client Assessments: Conduct assessments for current and potential clients to determine eligibility for services and appropriate care settings. Collaborate with Health Services and Assisted Living Directors to identify prospective clients. Service Coordination: Ensure accurate scheduling to meet resident needs, address service interruptions proactively, and maintain clear communication with team members and clients. Family & Resident Support: Consult with residents and families regarding care options, challenges, and resources. Develop and regularly review individualized service plans with residents. Financial Oversight: Create and manage the program budget to support growth and meet financial objectives. Ensure accurate and timely billing processes. Collaboration: Work closely with marketing and other departments to cultivate relationships and identify opportunities for program development.
