Baystate Health, a nationally recognized leader in healthcare quality and safety, and home to Baystate Home Health and Hospice is looking for a Care Coordinator to oversee the SNF at-home team.

The SNF - at- Home coordinator is a temporary grant-funded 14-month position responsible and accountable for planning, organizing, directing, and coordinating the clinical and administrative performance of the SNF- at-Home program. The coordinator collaborates with hospital case management, compliance, legal, finance, providers, and the entire SNF- at- Home team to ensure patient needs are met within the guidelines of the SNF - at- Home grant.

Job Responsibilities:

1) Coordinates clinical practice and deployment of resources, supplies, and DME to patient's homes prior to discharge from the hospital with hospital case management and the SNF@Home team.

2) Directly oversees the scheduling of all disciplines, including remote patient monitoring and provider visits.

3) Coordinates and participates in daily huddles and twice weekly IDT to discuss patients, care planning, and discharge planning

4) Monitors supply usage and ordering to ensure fiscal responsibility within the confines of the grant

5) Manages SNF @Home patient tracking spreadsheet and works with finance for proper billing of service

6) Assists and reviews patients with hospital case management and SNF@Home Provider prior to accepting the case

7) Coordinates elevated care with MIH provider, SNF@Home provider, Hospital, or SNF as needed for readmission to Hospital or at SNF if failing at home.

8) Makes home visits to patients for assessment as needed and requested.

9) Other duties as requested

Minimally Required Experience:

3 years of home care, clinical experience, or hospital case management.

Prefered Experience:

BSN and Clinical or operational supervisory experience

Assoc of Science in Nursing (Required), Bachelor of Nursing Degree
