Center for Human Development Caseworker in Springfield, Massachusetts

Caseworker Job Category: Professional Requisition Number: CASEW005452 Full-Time Caring Together Residential Program Springfield, MA 01118, USA Description The caseworker/manager will collaborate with the clinical staff around the completion of initial assessments, re-assessments, treatment planning, and up dates to treatment plans. The caseworker/manager will continually assess the youth\'s progress toward treatment plan goals and changes to services and treatment based on the amount of progress. The caseworker/manager will coordinate staff education around individual service plan goals, assist youth in making community linkages and mastery in independence skills. They are a liaison between the program, youth, families and external stakeholders to ensure an encapsulation of services. They maintain open communication, actively supports the team model and ensures all agency standards, policies, and procedures are utilized. Depending on the needs of the region, they need to maintain all certifications in order to assist on the floor. They need to have flexibility around schedule in order to meet the needs of the region/program. They are directly responsible to the Clinical Director/Assistant Director of Residential Services. Qualifications Education Required Bachelors or better. Preferred Bachelors or better in Social Work or related field.
