Wayfinders Housing Stabilization Case Manager in Springfield, Massachusetts

We are seeking a Housing Stabilization Case Manager to provide stabilization services to homeless families placed out of shelters into apartment units. The Housing Stabilization Case Manager will provide stabilization services that include tenancy preservation, financial education and making referrals for additional services. Responsibilities Include: Assessment and triage housing stability and case management; Conduct assessments of client housing needs and other barriers that affect stabilization including financial, legal, mental health, substance abuse, employment, health care and housekeeping; Review and complete monthly stabilization touchpoints. Develop and implement goals that address identified needs and maximize self sufficiency; Coordinate support services, initiate referrals, provide advocacy, crisis intervention and supportive counseling to clients; Develop partnerships with internal and external service providers to ensure comprehensive and seamless service delivery; Educate and assist families on financial literacy and obtaining needed resources; Enter and maintain ETO data used to track family progress on established goals; Conduct regular monthly contact with clients via home visits, phone, text, email and/or virtual platform (Zoom, TEAMS, Duo, etc.) and monitor family progress in meeting goals during stabilization; Maintain complete and accurate virtual case files in a timely manner; Assist in the mediation of issues between clients and owners/landlords; Assist client with struggles that directly impact tenancy preservation. Hiring Requirements/Preferences: Bachelor\'s Degree (4 years of college) in relevant subject matter such as social services or public administration; 3 years experience in housing or social services case management; Ability to organize and prioritize; Able to effectively communicate with all levels of internal staff and external clients and constituents; Valid Driver\'s License and reliable vehicle; CORI; Bi-lingual (Spanish/English) preferred; Working knowledge of PC based computer software. The wage range for this position is \$16.21 - \$19.45/hour, depending on education and experience.
