SUMMARY

The Manager of Referral Services promotes, facilitates, and oversees transition of care protocols for appropriate patients to the home health and hospice setting.

• Oversees a seamless and efficient referral process through effective communication with referral sources and clinical teams, while ensuring referral information meets regulatory requirements for home care and hospice.

• Responsible for creating a positive experience for patients and referral sources.

• Responsible for the oversight of all staff related to receiving and processing referrals as well as staff serving in a liaison and/or transitions of care role and customer service.

Required Work Experience:

• 3 years of home care or general nursing.

• Registered Nurse License in the state of Massachusetts.
