Springfield, MA
Provides physical therapy services to residents under the direct supervision of a physical therapist.
Provides physical therapy services to residents by assisting in the assessment, treatment, program planning and implementation, related documentation, and communication.

Education, Experience, and Licensure/Certifications
Must have an active state license or license eligible and meet the educational requirements as defined in the job description for that license type.
Associate Degree or Higher in from an accredited Physical Therapist Assistant program.
Responsibilities include but are not limited to the following:
Provides physical therapy services to residents under the direct supervision of a physical therapist.
Identifies indications for and assists in evaluations at appropriate intervals under the direction of the physical therapist.
Assists the physical therapist in identifying a problem list, short-term goals, long-term goals and treatment plan.
Maintains and completes accurate documentation, department records and reports.
