Description:

At Mercy Medical Center the Inpatient Care Coordinator unit provides quality care for patients by organizing patient care and treatments in incorporating all members of the care team. Care Coordinator helps patients seamlessly transition from one care area to another. Our work ethic is the cornerstone of all we do at Mercy Hospital. What you will do

In this critical role you will assist in creating and managing nursing care plans and will participate in the evaluation of current and future medical needs of the patient.

Minimum Qualifications

  • Education : Graduate of an accredited college or university affiliated nursing program. BSN graduate preferred.

  • Licensure : Current Licensure as a Registered Nurse RN in Massachusetts.

  • Certification : BLS (AHA/ARC). CCM or ACM Certification preferred.

Preferred Skills:

  • Education: Bachelor of Science in Nursing BSN degree preferred.

  • Experience: 3-5 years in Case Management, Utilization review, or similar experience preferred. Hospital or community-based experience preferred.

Work Schedule : Full time – 40 hours – Days
