Description:

Trinity Health Of New England is looking for a Licensed Certified Social Worker (LCSW ) to join our Ambulatory Oncology Team.

The Social Worker (LCSW) will support our two locations at Mercy Medical Center and Radiation Oncology in Enfield, CT. The LCSW will assesses and assists in addressing and meeting the psycho-social needs of cancer patients and families, enabling patients to utilize medical care and other supportive services to achieve their optimum level of health and function.

You Will Be Responsible For:

  • Assesses and counsels’ cancer patients/families referred to the Cancer Center.

  • Provides consultation to physicians and staff in ambulatory discharge planning, from treatment, survivorship services, to psychosocial distress assessment.

  • patient centered counseling and consultative responsibilities required based on individual needs assessment.

  • Communicates via written documentation in the clinical record and through verbal discussions, all pertinent information on patient status to ensure that the team understands patient’s circumstances relative to achieving the goals of the treatment plan and survivorship program.

Requirements:

  • Education: master’s degree Required

  • Valid social work license from the Commonwealth of Massachusetts , Licensed Certified Social Worker (LCSW) required.

  • Valid Social Worker License from the state of CT , Licensed Master of Social Work ( LMSW ) required.

  • Two (2) years of related professional experience required.

  • Ability to communicate effectively both orally and in writing. This may include explaining clinical, technical, and diagnostic procedures to clients and their families, counseling clients and their families and enforcing policies and regulations in difficult situations.

  • Work requires the analytical ability to collect information from diverse sources, apply professional principles in performing various analyses, and summarize the information and data to solve problems or design complex systems and programs.
