State of Massachusetts Human Services Coordinator I in Walpole, Massachusetts

The Department of Developmental Services (DDS) Metro Region is seeking a Human Service Coordinator I (HSC) with strong attributes in the Human Service field to work in the Newton South/Norfolk Area Office. The successful candidate for this position will work primarily with adults (age twenty-two and above) who have been determined eligible for DDS services. _Duties and Responsibilities (these duties are a general summary and not all inclusive):_ * Coordinate and facilitate development of an Individual Support Plan that incorporates a broad spectrum of the individuals needs, skills and desires. * Arrange for referrals of services and supports both within and outside of the DDS service delivery system as determined by the needs of the individual, and monitor those services. * Monitor a situation where an individual is at risk and arrange for crises intervention if appropriate. * Maintain the electronic consumer records in Meditech and HCSIS for assigned individuals. * Visit programs, individuals, family members, guardians, and agency support staff. * Collaborate closely with the other Area Office staff and provide information to the Area Office Management Team. * This position will also be a Support Broker whose job duties, through our Self-Direction program, include facilitating and arranging the budgeting and provision of self-directed services to individuals and families in the Newton/South Norfolk area. _ Please Note:_ * Driving, both within and outside of the Newton South/Norfolk catchment area, will be a necessary part of this position. * Schedule: Monday - Friday 8:45AM to 5:00PM, Days Off: Saturday and Sunday * Interested applicants should be flexible in schedule as there may be occasional nights and weekend meetings with individuals, guardians, families and other agencies. _Preferred Qualifications:_ * Ability to listen attentively and respectfully to ascertain the desires and needs of individuals. * Proficiency to facilitate groups with diverse representation. * Capacity to function effectively as member of a team. _About the Agency:_ The Department of Developmental Services is dedicated to creating, in partnership with others, innovative and genuine opportunities for individuals with intellectual disabilities to participate fully and meaningfully in, and contribute to, their communities as valued members. _Benefits and Compensation:_ As an employee of the Commonwealth of Massachusetts you are offered a great career opportunity influencing a wide-spectrum of services to the diverse populations we serve - but it's more than a paycheck. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver’s license at a class level specific to assignment. Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor’s degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.
