State of Massachusetts Human Services Coordinator II in Waltham, Massachusetts

The Department of Developmental Services (DDS) is filling the Human Services Coordinator II (HSC II) role in the Charles River West Area Office. The HSC II will provide leadership, supervision and support to a team of five Service Coordinators who facilitate the arranging and monitoring of DDS funded services to the individuals and families in the Charles River West Area. The selected candidate will maintain records and work within HCSIS, PAM and Meditech. Provide monitoring, review and supervision of ISP development and approval (including ensuring that ISP content accurately reflects all services) assign individuals to Service Coordinators, Participate in screening, interviewing and selection of candidate for HSC positions, train new Service Coordinators in job duties, conduct EPRS for HSC staff, participate in area planning and protocol development, work as a member of MASSCAP team for determination of prioritization of services. _Duties and Responsibilities (these duties are a general summary and not all inclusive):_ * Maintain records and work within HCSIS, PAM and Meditech. * Assure team of SC’s meet all waiver requirements and related activities as identified in their EPRS (i.e. site visits, HCSIS timelines, etc) * Provide monitoring, review and supervision of ISP development and approval (including ensuring that ISP content accurately reflects all services and other expenditures in the individuals' budget) * Train new Service Coordinators in job duties * Conduct EPRS for HSC staff * Participate in area planning and protocol development * Work as a member of MASSCAP team for determination of prioritization of services and conduct MASSCAP assessments. * Provide supervision and support to traditional Service Coordinators. * _PLEASE NOTE:_ * *_Schedule:_ Monday through Friday, 9:00am - 5:00pm; Days off: Sunday/Saturday* * /Flexibility required to meet the needs of the Area Office./ Will require a driver's license as there will be traveling involved with this position. _About the Agency:_ The Department of Developmental Services is dedicated to creating, in partnership with others, innovative and genuine opportunities for individuals with intellectual disabilities to participate fully and meaningfully in, and contribute to, their communities as valued members. _Benefits and Compensation:_ As an employee of the Commonwealth of Massachusetts you are offered a great career opportunity influencing a wide-spectrum of services to the diverse populations we serve - but it's more than a paycheck. Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor’s degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required. Based on assignment as a Mental Health Case Manager or Forensic Transition Team Coordinator, within the Department of Mental Health, a Bachelor’s degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation counseling, education or other a related field is required.
