In this position you will work with a dynamic team overseeing health care needs of 5 delightful social individuals with acquired brain injury. These individuals reside in a beautiful architecturally designed and handicapped accessible home with amenities to meet their needs with the goal of promoting a positive and fulfilling life. 24-hour care to include medication administration is provided by our direct care support staff. You will have an integral role along with an experienced RN as educator and resource to the direct support staff and team members. The LPN Care Coordinator will, on occasion, be required to meet with overnight staff in early AM (8 am, earlier if desired) for training, and to occasionally participate in staff meetings and accompany the individuals, sometimes remotely, to physician appointments (usually only on weekdays) Responsibilities: ? Ability to utilize the nursing process to promote optimal mental and physical health of our individuals ? Serve as a resource and educator in development of wellness plans based on diet, medication regimen, exercise, preventative care and education to promote healthier lifestyles ? Assist in ongoing training of direct support staff in caring for individual?s medical and personal care needs ? Act as a consultant responding to issues related to medication administration and acting as a resource to staff when changes in condition and urgent situations arise. Make subsequent recommendations for follow up. ? Collaborate with interdisciplinary team members, VNA, physicians, hospital case managers and ABI Department of Developmental Disabilities (DDS) and family members in developing and following an individualized plan of care . ? Actively participate in multidisciplinary team meetings ? Maintain communication and coordination with VNA for skilled nursing services, physical and occupational therapy for each individual ? Provide support and training to staff on interpretation of regulations, policies and procedures of DDS, DPH and WCI. Makes recommendations in order to meet compliance with regulations and provide optimal quality of care ? Provide medical case management including audits of medical records, training of support staff to interpret and follow up on medical appointments, coordination of services, implementation of recommendations and treatment ? Attend physician appointments on occasion and review physical exams and visits to specialists. Assist in ensuring that treatment plan is followed up and medical care, psychiatric care and referrals are coordinated ? Serve as advocate for individual members and develop positive relationships with Operations, families and significant others ? Coordinate admissions and discharges from hospitals, SNFs or rehab settings with other team members to ensure a seamless transition. Maintain documentation in medical record ? Develop individualized protocols and trains staff on providing care and health supports to individuals with complex medical conditions ? Assist in providing direct care to include med administration when needed ? Successful completion of on-line MAP trainer course w/in 3 months of hire ?
