The MAT Program Coordinator provides operational oversight of the Medication Assisted Treatment (MAT) Program within the Worcester County Jail and House of Corrections. The Program Coordinator is responsible for coordinating treatment and facilitating post release continuation of care planning as well as ensuring overall adherence to program standards.

Responsibilities
Ensure compliance with all governing regulatory bodies and program policies for MAT Program
Communicate and collaborate with providers and all members of the multidisciplinary health care team as needed to facilitate MAT intakes, clinical treatment, program adherence and continuation of care planning.
Participate in multidisciplinary treatment planning, case reviews, conferences and other meetings as directed.
Ensure completion of all required paperwork for existing and new MAT patients.
Support the MAT team with grant tracking, data entry and required reporting as well as developing/updating program policies.
Ensures that patients are treated with dignity and respect in accordance with Advocates Human Rights Policy.
Adheres to all principles related to the Advocates Way.
Performs all duties in accordance with the agency’s policies and procedures.
Qualifications
Licensed Social Worker or Mental Health Counselor or closely related license in the fields of addiction or mental health required
Minimum of two years direct experience in the substance use, mental health or related behavioral health program required
Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.
Must be able to perform each essential duty satisfactorily.

Ability to effectively communicate verbally and in writing.
Ability to utilize electronic health records efficiently and effectively in the provision and documentation of care.
