Summary This is an open continuous announcement (OCA) and will remain open until December 31, 2022. VA Boston is hiring for our Intensive Care Unit ( ICU) Float pool. Positions can range from per diem ( with minimum required shifts/weekends/holidays established), part time or full time. Hours will be discussed during interview. Responsibilities Evaluates nursing interventions based on patient's response and revises nursing care plan accordingly. Develops meaningful goals to assist the critically ill patient and family in attaining the optimum level of physiological, sociological and psychological balance within the limitations imposed by their illnesses. Assures documentation reflects assessment of patient needs, interdisciplinary treatment plans, clinical interventions, and evaluation of patients' responses to treatments. Interpreting cardiac arrhythmia and administer appropriate drugs per physician orders. Initiates IV's and approved IV medication administration and treatments, emergency and resuscitative measures based on appropriate standing orders and protocols. Administering therapeutic treatment measures such as medications, intravenous fluids, blood products, treatments and procedures and physician's in timely and complete manner. Performs timely titration of ordered medication according to established parameters and unit medication policy. Observes and assesses patient's clinical condition: recognizing, identifying and interpreting serious situations and immediately implementing proper action. Documents observations, nursing interventions, responses to therapeutic measures and status of coordinated activities between nursing and other profession disciplines. Operates and monitors specialized equipment used for hemodynamic monitoring, transcutaneous oxygen monitors, oximeters, ventilators, infusion pumps, syringe pumps, defibrillator, resuscitation units, respiratory therapy apparatus, monitoring devices (cardiac monitors, oxygen monitors, etc.), IV pumps and regulators, temperature regulating devices, suction, blood glucose monitors, mist tents, blood products etc. Prepares patient, completes or assists as appropriate during procedure and evaluating results. Assists the physicians in performing complex or potentially life-threatening procedures such as cardioversion and insertion of invasive lines, e.g. arterial and pulmonary artery catheters, chest tubes and endotracheal tubes. Uses the Computerized Patient Record System (CPRS), Bar Coded Medication Administration (BCMA) and PYXIS as applicable. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient, and family education, patient self-management after discharge, and supporting the factors that impact customer satisfaction. Completes assignments in a timely and effective manner and follows safety protocols designed to reduce/eliminate employee risk of injury. Identifies risk areas and analyzes resources available in the use of patient care equipment and body mechanics, while evaluating options available and selecting a course of action resulting in favorable patient/organizational outcomes. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Licensure: Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Minimum of 2 years' recent experience in ICU General knowledge and experience working the spectrum of nursing. Strong knowledge of the nursing process and critical thinking skills Ability to apply critical thinking in any environment Independent, and adaptable to working in any area assigned Strong technical skills including knowledge of computerized charting Flexibility schedule to meet patient care needs CCRN certification Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, bending, pulling, pushing, and sitting at a keyboard. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs and able to manage work activities in a highly technological integrated work environment.
