Mercy LIFE, a member of Trinity Health PACE, is seeking a Licensed Practical Nurse for its location in W. Springfield, MA. The Program of All-Inclusive Care for the Elderly (PACE) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible.

PACE serves individuals who are age 55 or older, certified by their state to need nursing home care, able to live safely in the community at the time of enrollment and live in a PACE service area. While all PACE participants must be certified to need nursing home care to enroll in PACE, only about 7 percent of PACE participants nationally reside in a nursing home. If a PACE enrollee needs nursing home care, the PACE program pays for it and continues to coordinate the enrollee's care. We encourage fellowship and independence while serving seniors and their families with dignity and respect. Delivering all needed medical and supportive services, PACE is able to provide the entire continuum of care and services to seniors with chronic needs while maintaining their independence in their home for as long as possible.

LPN – Licensed Practical Nurse

Position Purpose:

The primary purpose of this position is to provide oversight and management to the provision of service delivery to participants. Provide skilled nursing care and prescribed treatments to participants according to professional nursing standards and primary care orders.

What you will do:

  • Provides skilled nursing care/prescribed treatments to participants according to professional nursing standards and primary care orders.

  • Demonstrates nursing care to and instructs participants and families in treatments and other self-care interventions.

  • Participates in daily program activities and provides comprehensive plan of care to meet participant needs and promote optimum health, independence and self-care.

  • Communicate, counsels and guides participants and families regarding service needs and concerns; makes referrals as appropriate.

  • Assist participant/caregiver with filling of medication sets, administering medications sent by pharmacy and accurately completing documentation process.

Minimum Qualifications:

  • Must be a graduate of an accredited Nursing Degree program and possess a current, unencumbered, active license to practice as a Licensed Practical Nurse in the state of PACE Organization.

  • Active CPR Certification required.

  • Minimum of two (2) years of demonstrated successful experience in related services with geriatric population.

  • Must have the ability to perform standard clinical procedures according to established protocols and a high level of accuracy and thoroughness in documenting the delivery of services.

  • Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.

  • Superior written and verbal communication skills appropriate for audience comprehension. Able to communicate effectively with individuals and groups representing diverse perspectives.

  • Must be action-oriented, manage conflict well, be customer focused, have high decision quality and be flexibility to adapt to ongoing change.
