Wellforce PHO Navigator in Woburn, Massachusetts
Position Summary: Responsible for “navigating” patients through our healthcare system by scheduling appointments with specialists at the point of contact to provide expedited and convenient service, responding to patient inquiries on the Physician Finder Line and ensuring patients and offices are using the Patient Portal efficiently and effectively to provide optimal communication between patients and providers. Communicates regularly with Primary Care Physician (PCP) and Specialty Physician offices. Responsible for maintaining relationships with MWHC Emergency Room and affiliated Urgent Care Centers (Including Physician One Urgent Care) to ensure patients in need of a primary care provider or follow up specialty care are addressed. Responsible for the Physician Finder Line and following up with patients in need of primary or specialty care. Oversees the Patient Portal by assisting patients and offices with questions and issues and provides training to ensure optimal use of the portal. Responsible for following the PHO referral guidelines and working with the PCPs, PHO Medical Directors, office staff and the patient to help direct care, when medically appropriate, to a specialist within the PHO network and to MWHC services. Assists with various projects/services within the department on an “as needed” basis.
High School graduate or equivalent education required.
One to three years health care experience either in the areas of patient registration or physician office.
Knowledge of hospital and medical terminology. Experience with physician and hospital information systems. Excellent Customer Service skills. Knowledge and skill with Microsoft office.
