Description and Responsibilities

Open Sky Community Services, in collaboration with the Department of Corrections (DOC), is excited to announce a first of its kind program in the state of Massachusetts, The After Incarceration Support Program. This program will support the re-entry of returning citizens who have been recently released from the DOC and other individuals who have a history of Justice System Involvement. The Aftercare Incarceration Support Program is community-driven and fosters active community involvement, and engagement. The program offers a safe , judgement free, resource rich environment that breaks down barriers to reintegration while providing access to essential, customized resources, and supports a mission to invest in each individual’s success. The scope of services includes outreach and engagement, food and nutrition assistance, educational and job placement services, housing assistance as well as healthcare and addiction treatment.

The After Incarceration Support Program Navigators work with individuals served to coordinate access to services and assist in fulfilling integrated health needs including behavioral health and medical needs, as well as foster partnerships and collaborations to support the members of the program. They also provide In-Reach and Re-entry supports, assessment, education, and safety planning for individuals enrolled in the program.

Other Key Responsibilities:

  • Participate in the development and implementation of program goals, and objectives toward providing support to justice involved enrollees transitioning back to society and promoting successful community tenure.

  • Schedule and conduct outreach via in-reach visits. Utilize person-centered framework to identify the enrollee’s preferences, desired level of involvement and determine addressing any cultural considerations or interpreter services needed.

  • Identify referral sources for program members and conduct program presentations and outreach.

  • Engage with potential community partners to enhance the opportunities for members to engage in employment and community activities.

Qualifications

  • High School Diploma, GED or equivalent, required.

  • Valid driver's license, acceptable driving record, reliable and appropriately insured vehicle, required.

  • Experience with community-based behavioral health supports, substance use disorder services, justice involved population, and/or community re-entry programs, preferred.

  • Lived experience preferred.

  • Must meet participating Correctional Institutions requirements for facility entry.
