ANCHOR Care Manager (hybrid schedule) Multifaceted social service agency serving elderly and persons with disabilities, seeks ANCHOR Care Manager (Advocacy & Navigating Care in the Home with Ongoing Risks) to assess consumer needs, conduct home visits, develop service plans and maintain case records. Strong skill set working with individuals with complex LTSS needs and challenges as it relates to effective service delivery. Bachelor's degree required. Behavioral Health background preferred. Must possess computer skills. Use of a reliable vehicle including active vehicle insurance with a current license to drive required. Assist eligible elders with recurrent, fluctuating behavioral health needs, to obtain and retain services which will enhance their quality of life and allow them to live as independently as possible with dignity and respect. ANCHOR Care Manager will have a reduced combined caseload of Home Care and ANCHOR consumers to allow for more frequent contact between the ANCHOR consumer and the Care Manager. Care Manager will co-manage some ANCHOR cases with Protective Services/Crisis with the goal of working with the elder to accept/retain Home Care services to alleviate risk. Position will be supervised by a Home Care Supervisor and consultations with an in-house Behavioral Health Consultant will be provided to further support the Care Manager while promoting acceptance of services with consideration of the consumer's behavioral health needs.
