Description and Responsibilities
The Assertive Community Treatment program provides individualized, 24/7 care and continuity of care (open-ended, lifelong) to any person who needs it within the community. The program is an alternative to traditional outpatient treatment that was designed to mimic hospital-level wraparound support to help individuals maintain in the community, rather than bounce back and forth from the hospital. Our goal is to help people retain their independence in the community and help them live a life that is not defined by their illness. The outreach is assertive, and the team is highly integrated, multidisciplinary and persists in the face of failure. We meet individuals where they are at and bring our support to them by going to settings and spaces where they feel safe and comfortable. Our unique contract with the Columbia Center for Practice Innovations provides the program robust, job-specific training to each team member.
The Registered Nurse is a member of a multidisciplinary team and aims to understand, embrace, and operate within the ACT model. The registered nurse is responsible for conducting psychiatric assessments, assessing physical health needs, making appropriate referrals to community physicians, providing management and administration of medication in conjunction with the psychiatrist, providing a range of treatment, rehabilitation, and support services, and sharing shift-management responsibility with other staff. The registered nurse collaborates with the team, the individuals enrolled in services, and their collaterals to develop and revise comprehensive assessments and individualized treatment plans. Other Key Responsibilities:
In collaboration with the psychiatrist, develop, revise, maintain, and supervise team psychopharmacologic and medical treatment and medication policies and procedures including transcribing, administering, evaluating, and recording psychotropic medications prescribed by the team psychiatrist
Evaluate and document psychotropic medication effectiveness, complications, and side effects
Provide case management and care coordination support including making referrals and building and maintaining relationships with health services providers in the community (e.g., community health centers, detox facilities, inpatient psychiatric and medical units, etc.)
Provide clinical and non-clinical support in the form of psychoeducation to individuals and families, educational and vocational supportive counseling, supporting individuals to maintain safe and stable housing, teaching life skills, and symptom management
Participate in the on-call rotation
Must be a licensed Registered Nurse in the state of Massachusetts.
Minimum of 1 year expereience working with adults with serious and persistent mental illness and substance use disorders, required.
Experience with local resources, navigating health, behavioral health, and/or substance abuse treatment systems, required.
Valid driver’s license, acceptable driving history and reliable transportation, required.
