Community Healthlink Case Manager in Worcester, Massachusetts
Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Case Manager in Worcester, MA.
Community Healthlink’s Community Support Program (CSP) provides short term, intensive outreach, and case management services to support individuals at risk of repeated psychiatric hospitalizations and/or inpatient substance abuse treatment admissions. CSP services are flexible and based on individual needs but focus primarily on helping clients overcome challenges with establishing and maintaining access to health care, community-based resources, and social/peer supports.
CSP case managers meet with referred clients in a variety of community settings, complete intake assessments and person-centered service plans, then assist clients with achieving the goals identified in those plans. CSP duties include researching community-based resources and assisting clients to connect with these resources by helping with things such as short-term transportation, advocacy, encouragement, skills building, coaching, and completing paperwork.
Develop and maintain rapport with individuals referred to CSP.
Provide telephone outreach and home/community visits if needed
Provide “hands-on” case management support to clients, including providing short-term transportation to support engagement with providers and social services
Link individuals to resources and valuable information to improve health and wellness.
Help advocate for individuals to improve their access to care.
Complete required written documentation per program expectations.

Grade C04: Requires a Bachelor’s degree; preferably in the field of psychology or human services
Experience working with addictions and mental health issues
Strong communication and listening skills
Comfortable working independently and within a group
Flexible yet organized
Bilingual (Spanish/English or Vietnamese/English) a plus
Must have a current valid US-issued driver's license and ability to provide registered, inspected, and insured automobile for work related purposes including transporting clients in your own vehicle.
