Community Healthlink Case Manager/Therapeutic Mentor/Therapeutic Training & Support Staff in Worcester, Massachusetts

Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a Full-Time (37.5 hrs/wk) Therapeutic Support & Training (TT&S) and Therapeutic Mentor (TM) at our Worcester site.

The In-Home Therapy team provides strength-based therapy to families, children, and youth under 21 years old who have significant behavioral health needs. The TT&S works in collaboration with a Master’s level clinician to provide therapeutic interventions in the home/community setting with the purpose of improving the youth and family’s functioning in the home setting. The TM provides 1:1 skill based intervention to a youth under 21 in the community setting with the purpose of improve their overall functioning in community settings. The mentor works with the youth to strengthen variety of skills ranging from social skills to independent living skills and improve overall social functioning at home, school and in the community.

This position is wonderful opportunity to have experience working with clinicians doing intense family work and working with children/youth one on one teaching them skills, as well as exposure to working with multiple behavioral and child welfare systems.   • Ability to work as TT&S under the supervision of a Master’s level clinician to support the implementation of a treatment plan for the youth and their families, which would include coaching and teaching coping and emotional regulation skills.

  • Provide weekly TM sessions that include teaching, coaching and supporting development of a range of skills in youth.

  • Complete paperwork that includes Intervention Plans and discharge summaries

  • Maintain client records (including completing progress notes).

  • Communicate with collaterals for the purpose of treatment planning

  • Participate in wrap-around collaboration and community support meetings.

  • Maintains unit productivity standards.

  • Attends staff supervision, meetings and trainings as required.

  • Follow CHL protocols.

  • Some evening and weekends may be required.

  • Travel is required as part of this position

  • Grade 5: Case Manager I: Associates degree required

  • Grade 6: Case Manager II:Bachelor’s Degree-Human Service/Psychology or related field required.

  • Minimum of two years working with children, adolescents and families with significant behavioral, social and/or emotional difficulties.

  • Proven ability to effectively engage clients, successfully advocate for clients and to work well with other professionals.

  • Comfortable providing home-based and community based services.

  • Demonstrated strong work ethic, ability to work independently and ability to contribute to team objectives

  • Demonstrated strong communication skills

  • Ability to use a computer-based information system.

  • Current valid US-issued driver’s license and ability to provide registered, inspected and insured automobile for work-related purposes including transporting clients in your own vehicle

  • Must be able to pass a CORI background check
