Community Healthlink Director of Housing and Community Support in Worcester, Massachusetts

Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Director of Housing and Community Support in Worcester and Northern Worcester County, MA.

The Tenancy Preservation Program (TPP) provides a clinical presence in the Central MA Division of the Housing Courts and short-term intensive case management to eligible households whose tenancy problems are related to a disability. Our primary responsibility is to the Housing Court, providing professional assessment and recommendations regarding the disability-related causes of the tenancy problems and recommendations for service plans and other reasonable accommodations to address the tenancy problems.

  • Oversight of operations of Community Support Program (CSP); Tenancy Preservation Program (TPP); and the HOAP Housing Programs.

  • Provide supervision to the managers and supervisors of the 3 programs.

  • Ensure that the HR functions are provided and CHL guidelines followed for each program including hiring, orientation, training, discipline and performance evaluation.

  • Maintain relationships and effective communication with the payers of the programs including but not limited to MassHealth, ACOs/MCOs, DMH and CMHA.

  • Provide group and direct one on one supervision to the CSP/CSPECH Teams as needed and/or required by the payor.

  • Ensure that staff are trained and coached in completing needs assessments, strength based, person centered service plans, progress notes, and discharge summaries according to the payer specifications.

  • Develop & maintain relationships with both internal and external referring providers and payors.

  • Maintain working knowledge of reimbursement policies and procedures for all payors.

  • Oversee reporting, billing, and contract monitoring of the programs in conjunction with the program managers and supervisors.

  • Demonstrate effective communication with consumers, staff, peers and other professionals.

  • Perform duties using recovery-oriented principles.

  • Model person centered, strength-based communication with staff and consumers.

  • Assist managers in developing connections with community-based resources and referral networks.

  • Work with program management on Quality Improvement initiatives including but not limited to developing score card, idea boards and PDSA quality improvement projects

  • Participate in Coordination of Care (CoC) referrals and meetings including presenting cases, interviewing applicants, and monitoring filled, and open subsidies held by CHL.

  • Document homeless status for clients or potential clients in Clarity system.

  • Perform other related duties.

  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.

  • Must have a Master’s degree and an independent license in MA (LICSW, LMHC, Licensed Psychologist).

  • The ideal candidate will have experience working in a community healthcare setting and have a working knowledge of mental illness and substance use disorders, as well as knowledge of models of care integration and care management.

  • Must have a minimum of two years’ experience.

  • Previous supervisory experience required.

  • Possess strong interpersonal skills and confidence.

  • Able to exercise good judgment and is creative and proactive.

  • Be flexible and able to multi-task.

  • Current valid US-issued driver's license and ability to provide a registered, inspected and insured automobile for work related purposes including travel between work sites.
