Community Healthlink Outreach-Triage Case Manager-Primary Care in Worcester, Massachusetts

Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Outreach/Triage Case Manager in Worcester and north Worcester County.

CHL's Primary Care practice provides primary care, behavioral health care and substance use treatment in an integrated behavioral health setting. Our highly qualified, trauma-informed providers utilize clinicians and nurse care managers to deliver a person-centered team approach to meet the medical and behavioral health needs of each patient through individual and group treatment.

The Outreach/Triage Case Manager provides all needed levels of supported referral to benefits, services and coordination of other essential services (i.e., housing) to homeless adults seeking assistance.

Weekly clinical supervision to meet clinical licensing requirements

  • Perform comprehensive intake and assessment of the supportive service, clinical, emergency shelter and/or housing needs of homeless adults.

  • Develop appropriate service plans in conjunction with clients for the delivery of essential services.

  • Work collaboratively with primary care and behavioral health multi-disciplinary team to coordinate the delivery of appropriate clinical and other support services.

  • Make appropriate referrals to existing community agencies for additional supportive or housing services as needed within the Worcester community.

  • Assist clients with obtaining various entitlements and benefits (i.e., SSI, EAEDC, Mass Health).

  • Provide transportation assistance to homeless individuals as needed.

  • Attend relevant case management training activities.

  • Make referrals and assist clients with applications for subsidized (Worcester Housing Authority) housing, sober housing or other housing resources.

  • Participate in collaborative case conferences regarding service planning for homeless adults.

  • Regularly attend weekly staff meetings, clinical team, supervision and all other required program meetings.

  • Identify gaps in homeless services and provide data for the purpose of client advocacy.

  • Prepare and maintain confidential client documentation, service records, and preparation of statistical reports.

  • Assist clients with all benefit related concerns

  • Perform all other related job duties as required.

  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.

  • Grade 5-Case Manager I: HSD/HiSet/GED required; Associates degree preferred

  • Grade 6- Case Manager II: Bachelors degree required; degree in Social Work, Psychology, Counseling or related field preferred

  • Demonstrated knowledge base of homeless population and relevant community resources.

  • Ability to coordinate all appropriate client services.

  • Ability to effectively communicate with consumers, peers and other professionals.

  • Timely and accurate documentation skills.

  • Bilingual/Bicultural, Spanish helpful.

  • Knowledge of homelessness and related issues.

  • Familiarity with community resources and entitlements (i.e., substance abuse, mental health).

  • Current valid US-issued driver's license and ability to provide a registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle

  • Must be able to pass a CORI background check
