We are currently hiring a Population Health Manager based in Worcester. Population Health Manager works with organization-wide leadership to improve clinical quality while reducing the total cost of care. Partner with health center staff to promote excellent health care to the entire patient population, particularly when it comes to gaps in care (quality) and capturing patients’ health status (coding and social drivers of health).

Essential Functions:
Play a critical role in performance management by identifying gaps and root cause operational issues, both through data and operational observation, by providing feedback and education, and by guiding actionable initiatives.
Assist the team to sustain the success in localizing the Model of Care and the goals of the Model of Care.
Work with team to sustain and improve Complete Coding programs and achieve annual program goals.
Work with the team to sustain and improve pay-for-performance programs and achieve annual program goals.
Lead the transition of innovative ideas from pilot to implementation and share best practices with other health centers.
Assist with various aspects of managing the Model of Care, including specialty referral strategies, co-management agreements, provider meetings, improving enrollee access and improving enrollee experience.
On a regularly established schedule, review financial (cost and utilization) and quality performance reports.
Education and Experience:
Minimum 2 years of leadership experience in project management and practice re-design work including Patient Centered Medical Home, Practice Transformation, Quality Improvement, ACO development, Ambulatory Care, Quality and Efficiency metrics, and EMR
Working knowledge of clinical quality metrics such as RAF, HEDIS, NCQA or National Quality Forum type metrics
Experience in data analytics comprehension
Exceptional communication skills, both written and oral, ability to positively influence others with respect and compassion
Excellent written and oral presentation skills, with the ability to engage, inspire, build credibility and trust
Highly organized and self-motivated individual with ability to adapt to various workspaces and work autonomously
Collaborative working style with the ability to work across different teams, areas of expertise, and adapt to ambiguous environments and clientele
Ability to identify trends, predict issues, highlight critical areas, and develop corrective action plans
Must have a valid Driver's License and ability to travel.
Proficient in Microsoft Office products (Excel and PowerPoint)
