Community Healthlink Program Coordinator- Chelsea House in Worcester, Massachusetts
Come join a friendly and diverse clinical team that provides quality care! Community Healthlink (CHL) is looking for a full-time (37.5 hrs/wk) Program Coordinator in Worcester, MA.
The Chelsea Street program provides group home housing for eight adults with persistent mental illness along with various medical conditions and/or current or previous substance abuse. The Chelsea Street Program focuses on teaching individuals how to become more independent with activities of daily living such as cooking, cleaning, budgeting, and taking their medications. The program is located within a conveniently located Worcester neighborhood and offers many opportunities for our individuals to lead more independent lives. Chelsea Street is staffed 24 hours per day, including awake overnight staffing.
The Program Coordinator provides administrative supervision of staff as required and assists with implementing interventions for individuals using Evidence Based Practice strategies. They will also collaborate with the Integrated Team members and Directors to provide clinical support and consultation around Persons who have complex psychiatric and/or medical needs and who present with heightened symptoms and increased behavioral risks.

Supervise, support, and/or mentor Licensed staff members and other staff as required.
Facilitate and participate in team meetings to coordinate enrollment of new Persons to ACCS and/or transition or dis-enrollment/discharge from services.
Work with other licensed practitioners to complete initial assessments or comprehensive assessments to determine persons individual clinical and rehab needs.
Work with other licensed practitioners to develop Treatment Plans and Clinical Interventions.
Effectively communicate with all members of the team to ensure that services are delivered in a collaborative, clinically integrated, strength-based, culturally responsive and recovery orientated manner.
Ensure that services are person-centered and truly encourages the persons participation in his/her own recovery.
Collaborate with the Integrated Team members and Directors to provide clinical support and consultation around Persons who have complex medical needs and who present with heightened symptoms and increased behavioral risks.
Work with Peer Specialists to ensure that peer-to-peer counseling and self-help options (particularly WRAP) are promoted and integrated into the ACCS system.
Facilitate Treatment Team meetings with team members present.
Will provide transportation to persons as needed. May include transporting individuals in personal or program vehicle.
Provides administrative Supervision of staff as required.
Perform other related job duties.

Requires a Bachelor’s degree; preferably in the field of psychology or human services
Prefer a minimum of 2 years experience in the mental health field
Prefer a minimum of 1 years experience in management
Current valid US-issued drivers license and ability to provide a registered, inspected and insured automobile for work related purposes including transporting clients in a personal vehicle or a company van
Must be able to pass a CORI background check
