Job Summary:

To work in and oversee Kennedy Community Health’s School-Based Health Centers’ (SBHC) six sites. This role will promote quality, efficiency, and integration of services between our primary clinical sites and the SBHCs. To coordinate SBHC services with those offered at Kennedy Community Health. To enhance collaboration between Community Programs adolescent services and the SBHCs. 

To ensure SBHC providers meet expected productivity. To provide comprehensive primary care services to patients within an assigned outpatient primary care and/or school-based health center location and coordinate care of patients under the supervision of a Board Certified physician (NP/PA).

Essential Functions:

SBHC Management:

1.  Oversee the day-to-day operations of assigned School-Based Health Center sites to ensure that efficient and high quality systems of care are in place. Supervise the clerical staff at each assigned SBHC site. Facilitate the development of a team at each site that includes the school nurse, the medical provider, the school adjustment counselor, the Front Desk Staff, Psychotherapist, Community Health Worker, and dental hygienist.

2.  With the SBHC assigned Nurse Practitioners/Physician Assistants, develop programs to address health issues among the student body, inclusive of classroom-based health education, health education/counseling programs for students with specific health care needs, and expansion of the SBHC model to include mental health, dental, nutrition, and other types of clinical services at some schools.

3.  Quality: Assure that ongoing quality measurement and improvement activities are carried out in accordance with the Quality Improvement Plan for the SBHCs. Establish and maintain data collection systems in order to measure adherence to internally and externally determined objectives. Prepare reports for funding agencies as necessary.

4.  Development: Assure that all service and reporting requirements related to grant funding are met. Prepare all reports for funders in collaboration with the Development staff.   1. Develop and maintain relationships with school personnel, students, and parents to assure that the SBHC is responsive to student needs. Participate in School Health Advisory Meetings, Parent-Teacher Meetings, and faculty meetings as required in order to educate important constituencies about SBHC issues

  2. Represent Kennedy CHC at meetings with Worcester and Framingham Public Schools and other SBHCs in Worcester, Metro West and the state.

  3. Continually update procedures, policies, and protocols at the SBHCs. Perform other duties and work assignments as assigned by the supervisor including attendance of requested department, center, and outside meetings and/or trainings.

Clinical: (If Applicable)

1.  Deliver direct Primary Care to assigned patients. Develop therapeutic plans of care for prevention, management and maintenance of optimum health for individuals.

2.  Provide Preventive Health Services according to established practice standards. Provide health education and counseling to assigned patients by promoting positive health behaviors and self-care skills. Provide family planning counseling.

  1. Provide care for Acute and Chronic Illness and Urgent Care services that includes assessment, diagnosis, treatment and monitoring of illness.

  2. Consult with physician partner and/or refer to subspecialty care patients with more complex/dynamic problems.

  3. Provide primary care and preventive services to children and adolescent populations within the assigned site/location as assigned by the designated clinical supervisor.

  4. Administrative: Maintain and update Electronic Health Records (EHR) in a systematic format and complete required documentation within the specified amount of time expected by the assigned manager/director. Ensure that patient records are kept confidential consistent with Health Center policies and procedures, Joint Commission, and HIPAA standards. Monitors HIPAA compliance within the work environment.

Additional Clinical Responsibilities:

  1. Adhere to established protocols, responds to medical emergencies within the facility assigned.

  2. Assists/instructs other health care employees in the management of client care.

  3. Make referrals to health care professionals, specialists, other clinics and community agencies.

  4. Provide Clinical Supervision and mentoring to Nurse Practitioner, Medical and Physician Assistant students by mutual agreement with the Clinical Director.

  • Comply with all Health Center policies and procedures, while upholding the high expectations of EMKCHC.

  • Knowledge of the principles and practices of employee development sufficient to ensure organization productivity.

  • Ability to exercise a high degree of initiative, judgment, discretion, and decision-making to achieve organizational objectives.

  • Skill in analyzing situations accurately and taking effective actions.

  • Skill in establishing and maintaining effective working relationships with physicians, employees, and the public.

     

    Required Qualifications

  •  Master’s Degree in Nursing, Public Health or related field or equivalent work experience

  •  Clinical applicants must possess active and unrestricted Massachusetts licenses and certification as an Advanced Practice Nurse or Physician Assistant, Family Practice preferred

  • Strong knowledge of operational issues related to SBHC and/ or community health centers

  • Experience with program planning and strategic development 

  • Experience with quality improvement, care coordination and electronic health records

  • Knowledge of needs and issues effecting the health status of adolescents and children

  • Minimum 3 years supervisory experience

  • Excellent communication, administrative and organizational skills
