Veterans Inc., the largest provider of services to veterans and their families throughout New England, is looking for a professional, highly motivated, goal-oriented, skilled, and hardworking individual to fill the Street Outreach Case Worker position and become a valued member of Veterans Inc. team!

The Street Outreach Case Worker is a community-based position designed to seek out veterans and military service members who are actively homeless and provide them with information about COVID-19 resources and supplies, and encourage them to enter into a housing program provided by Veterans Inc.The goal is to provide access to care as seamlessly as possible, and with no obstacles that delay or confuse the veteran's request for screening, treatment, housing, and admission to other Veterans Inc. programs.

This individual would be based out of Headquarters in Worcester, MA. This position requires frequent travel, all duties would be performed throughout the designated territory's surrounding cities and towns.

This is a full-time position with full benefits (health, dental, vision, 403B, paid time off, short/long term disability and life insurance, etc.)

Must be able to pass a CORI and background investigation. Must be able to start within two weeks of receiving a job offer.

Essential Functions

  • Perform extensive outreach as a team to seek unsheltered homeless veterans who are at risk of COVID-19.

  • Target outreach areas include shelters, continuum of care providers, homeless camps, vacated buildings, and deserted areas that may be inhabited by homeless individuals.

  • Work closely with community providers and other homeless service providers to locate homeless individuals within the community.

  • Build relationships and encourage homeless veterans to accept shelter and quarantine services offered by Veterans Inc.

  • Provide resources and supplies to homeless veterans including those who are not eligible for VA services.

  • Refers clients who do not qualify for internal resources through the established community network.

  • Acts as a liaison between the homeless veteran and program staff and nurses when the client is admitted to the program.

  • Seamlessly provides internal referrals to Veterans Inc. mental health and substance use treatment services if desired.

  • The ability to demonstrate care and develop significant trust with clientswhile maintaining professional distance and objectivity.

  • Actively attends community meetings, activities, and events to become an contributing member of the homelessness provider network.

  • Utilizing the Homeless Management Information System (HMIS) for accurate recording and data entry.

  • Work one-on-one with individuals to provide linkages to health education; assistance applying for benefits including but not limited to health insurance, WIC, DTA services, legal, and child care.

  • Provides outreach services including eligibility determinations, needs assessment and assistance, skills enhancement training & transition, plan coordination to veterans through individual contact in open settings and outreach sites.

  • Conducts evaluations and assessments of all participants identified through the outreach process.

  • Maintains audit-ready, ongoing clients files, data collection, and reporting as required.

  • Attends and participates in all Veterans Inc. and external meetings as required.

  • Maintains a positive relationship with the clients, staff volunteers and visitors.

  • Performs special projects and other duties as assigned.

    Education and Experience Requirements:

  • Bachelor's degree in Human Services, Social Work or other related field required.

  • Minimum of two years' experience in Employment Training, Vocational Rehabilitation, Recruitment, Counseling, Outreach/and or Services to the Homeless, or related field, is required.

  • Computer proficiency in Microsoft Office Suite.
